the Service or (ii) a separate finders fee when we provide the Facilities with information about you as a sales "lead". WHILE WE ATTEMPT
TO VERIFY THAT THE INFORMATION PROVIDED BY FACILITIES LISTED ON THE SERVICE IS ACCURATE, WE ARE A FREE SERVICE FOR YOU INTENDED ONLY TO
PROVIDE YOU WITH AN INITIAL MEANS OF LEARNING ABOUT FACILITIES AND YOU ACKNOWLEDGE AND AGREE THAT, IF YOU REGISTER FOR THE SERVICE, IT
IS YOUR RESPONSIBILITY TO SEPARATELY REVIEW ANY INFORMATION ABOUT THE FACILITIES AS MAY BE LISTED ON THE SEPARATE FACILITY WEBSITE AND IN
CONNECTION WITH ANY CARE PROGRAMS OFFERED BY THE FACILITIES, AND TO REQUEST ADDITIONAL INFORMATION FROM, AND PERFORM YOUR OWN DUE

ARE SUBJECT TO THE SPECIFIC TERMS AND CONDITIONS AS LISTED ON EACH FACILITY WEBSITE AND AS ELSEWHERE PROVIDED TO YOU BY THE FACILITY.

Residence Required. You must sign a new lease and move into the care home. Then the care home must validate the following two items:
Your name appears on the lease agreement as an occupant; and
You have actually lived in the unit for more than 31 days.
Property on SeniorINFO.com. To qualify to receive a Reward, the property must be listed on SeniorInfo.com and you must find the
property on Seniorinfo.com. Please refer below to see the additional details. Residents will not qualify to receive a Reward if:
Residents who may have previously lived in the property listed on the move-in report form.
Residents who may have previously lived in a property that is owned or managed by the same company as the property listed on the lease report form.
Residents who are currently or have ever been or have family members who are currently or have ever been employees of the
company that owns or manages the property listed on the move-in report form.
Residents that move-in to a care home and vacate prior to 31 days.
Residents who find their care home using other websites or other referral services.
Confirmation by SeniorInfo.com. SeniorInfo.com must be able to confirm your use of our website. When you sign in to your account, we keep track of which listings you've seen.
When you submit a move-in report form, we look at your account to make sure you viewed the listing for the property where you've signed an agreement. If you have more than
one account, you must be able to provide SeniorInfo.com with the account you used to view the listing of the care home. We must be able to confirm the agreement and
occupancy with the new landlord.
2. Tell the Admissions Office/Management/Ownership That You Found the Property on "SeniorInfo.com."
When visiting a property, have the person who you are
touring with note that you found the property on "SeniorInfo.com." The staff or landlord may ask for your name and contact information-this is the perfect time to mention
that you found the property on SeniorInfo.com. In addition, you should note "SeniorInfo.com" as your source wherever possible: on the guest card, lease application, and
any other paper or electronic documentation. This helps us verify your use of SeniorInfo.com to find the property.
3. Report Your Move-In by Completing SeniorInfo.com's Lease Report Form to Claim Your Reward.
Signed a New Agreement. You must have signed a new agreement at a property you found on SeniorInfo.com. If you (or your loved one) has signed a previous lease agreement,
has a previous relationship with the property, or is merely renewing an existing lease, you will not qualify to receive a Reward. Only one Reward will be granted per lease signed or move-in.
Submission Deadline. You must submit your lease report form within 60 days after moving to the property. However, if the property has been terminated from SeniorInfo.com, you must
submit your move-in report form within 30 days after property termination. You can also submit your lease report form prior to move-in.
Expected Timing for Receipt of Reward Card. Processing the lease report form usually takes 30-45 days days from your move-in date or the day you report your lease, whichever is later.
